JOB TITLE: Resident Care Manager (RCM)
REPORTS TO: Director of Nursing
JOB SUMMARY: The Resident Care Manager (RCM) is responsible for coordinating all resident care services to ensure that all residents needs are met.
DUTIES AND RESPONSIBILITIES
· Acts as chairperson of the Interdisciplinary Care Plan Team.
· Completes incident and accident investigation.
· Ensures that each portion of the assessment is signed and dated by the person completing that portion of the MDS.
· Assists the Safety Committee in developing safety standards for departments.
· Determines nursing staff capacity for providing nursing care for any person seeking admission to the Community.
· Identifies condition changes and complex medical add-on residents. Make rounds with specific focus twice daily.
· Charts nurses’ notes in an informative and descriptive manner that reflects the care provided to the resident as well as the resident’s response to the care.
· Consults with the resident’s physician in providing the resident’s care, treatment, rehabilitation, etc. as necessary.
· Assists in developing diet plans for individual residents.
· Coordinates isolation procedures with nursing service.
· Ensures that documentation of the medication error includes a written error report signed by an observer/witness and the Resident Care Manager
· Assists, develops, and implements procedures and/or programs of the Community that seek to determine when, where and why medication errors occurring and how these errors can be prevented.
· Assures accuracy of MDS transmissions and QI information
· Carries out restorative and rehabilitative programs, progress, and report changes.
· Ensures that the care plan identifies any special equipment and utensils the resident may need (e.g. plate guard, enlarge silverware handles, etc.)
· Assists in planning regular and special diet menus as prescribed by the attending physician.
· Encourages attending physicians to review treatment plans, record and sign their orders, progress notes etc. in accordance with established policies.
· Ensures that appropriate documentation concerning unauthorized discharges is entered in the resident’s medical record in accordance with established procedures.
· Assists in planning the resident’s discharge plan as necessary.
· Transcribe physician’s orders to resident charts, cardex, medication cards, treatment/care plans as required.
· Assists community directors and supervisors in scheduling the resident assessment/care plan meetings.
· Perform other duties as assigned by the Administrator.
DUTIES AND RESPONSIBILITIES: IDT COMMITTEE MEMBER
· Assists with decisions concerning resident admission to the Community.
· Conducts and coordinates the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines that govern the resident assessment including the implementation of RAP’s and Triggers.
· Conducts or coordinates the interviewing of each resident for the resident’s assessment.
· Coordinates the review and revision of the resident’s care plan by the interdisciplinary team after each quarterly review or other assessment. Assures that the care plan is evaluated and revised each time an assessment is done or when there is a change in the resident’s assessment.
· Ensures that a completed RAI is done in designated time frame.
· Ensures that all staff are aware of the care plan and that care plans are used in planning daily dietary services for the resident.
· Ensures that all members of the assessment team are aware of the importance of completeness and accuracy in their assessment functions and that they are aware of the penalties, including civil money penalties, for false certification.
· Ensures that each resident receives the necessary nursing, medical, and psychosocial services to attain and maintain the highest possible mental and physical functional status as define by the comprehensive assessment and care plan.
· Involves the resident/family in planning objectives and goals for the resident.
· Reviews care plans to determine if changes in the resident’s care routine have been made on the care plan.
· Attends resident care planning conferences.
· Assures care planning updates whether temporary or long term.
· Ensures that all entries in the clinical record are informative and descriptive of the services provided.
· Documents resident’s mental, emotional, and physical status.
· Be familiar with the meaning and usage of common medical abbreviations, symbols, and terms relative to the administration of medications as used by the Community, pharmacists, and physicians.
Job Type: Full-time
Pay: $40.00 – $46.75 per hour
